Is it possible not to disclose personal health information to CIC? Would showing the dates of service, charges and the doctor's name be sufficient? Even though the personal claim history is exactly that, I am not sure what individual hospitals and clinics provide and what CIC requires. Thank you.

The information OHIP provides CIC does not mention anything specific. It just informs them when you used health services and the location. CIC request letter specifically asks you to contact Health services to get the information.

There is a box to be ticked that this request is for citizenship purposes. When you tick that, they send the information within 2 weeks.
