Received Medical Exam Form from CIC -YOB Error

Hi,anybody help because my wife received a CIC medical form under family sponsor class.
I sponsored her.
When we're going to print the form, the year of birth is incorrect.
I read alot of forum that encounter this problem, they said that ask the DMP to correct the error, just provide the correct information (i.e. passport, driver license , WP).
And my wife phoned already the CIC regarding to this matter and same advice the I.O said to her.
Here is the situation, i request Foss Note in CIC because our file has been transfered here in CIC Edmonton.
CIC sent me an email regarding to the Foss Note that i requested. The file that i received is GCMS Information Request. Sponsor Application. As i reviewed the sponsor application i noticed a error, it is the year of birth of my wife.
But on the Note 6. Of sponsor application, there is a text indicated the correct year of birth of my wife.
I dont know what are we going to do.
Are we going to send a letter that indicates the correct year of birth and provide evidence on that, then send to CIC?
Please advise.
