To apply for a Health Card (OHIP) you need 3 types of documents. You must bring original documents. You cannot use photocopies.

You cannot use the same document twice. For example, if you use your Driver's Licence for section 2, you cannot use it again for section 3. You may need additional documents. For example, if you have changed your name, you may need to show a marriage certificate or a change of name certificate.


According to a representative at Service Ontario, electronic statements or photocopies of electronic statements will not be accepted for health card renewal.

Because of the COVID-19 outbreak, Ontario is extending validation periods for driver licenses and OHIP cards. This means expired and expiring health cards will be accepted until the current situation improves. To apply for a new photo health card, you may have to visit a local Service Ontario centre and be aware of possible closures.

Thank you for sharing your situation and question with us.

You may renew your health card online here.
Before you renew, make sure you have:
  • your most recent health card
  • your current driver’s licence
  • a valid credit card (Visa, Mastercard) or Interac® Online (if renewing your driver’s licence)
  • a printer to download and print your temporary documents

To prove you renewed online, it’s important that you download, print and carry the temporary document at the end of your transaction, until your new cards arrive in the mail.

If you do not have access to a printer at the time of your renewal, you can download and save the temporary document and print it later.


Due to the current situation with COVID-19, expired health cards will also be accepted by Ontario health care providers.

According to Service Ontario’s COVID-19 notice:
Please note: we have extended validation to expired and expiring health cards. You do not need to renew at this time.


Until further notice, “the Ministry [of Health] requests hospitals provide all medically necessary services to all persons who present at a hospital, regardless of whether the person presents a valid OHIP card, or does not have health insurance under OHIP or another provincial health insurance plan.”

If you have an expired or an expiring health card, you may continue to use that card to access health services. Feel free to also contact Service Ontario’s INFOline at 1-866-532-3161 for more information.
