How do I renew a health card without a license?

I am trying to renew my Moms Health card. She doesn't have an ID card or a license. She also isn't very mobile so it's hard for me to get her out of her apartment

You may want to request that a physician or nurse practitioner fill out a Health Card Medical Exemption Request form. If she is over the age of 80 there are other options for renewing your documents as well. It may be best to call ServiceOntario at 1-866-532-3161 to go over your options and for case-specific advice regarding health card renewal.
