When filling the form for my adopted child who is declared under "accompanied family member" for our family sponsorship application, should additional family be filled as indicated? the adoptive parents to be parents or the child's biological parents?

Thank you for sharing your questions with us! We are happy to provide a high-level answer to your question.
In order to sponsor an adopted child, you must complete a sponsorship application that is separate from the family sponsorship application. You can find the application here.

You would have to obtain information for the form of Medical Condition Statement [IMM 0133] concerning the adopted child’s medical condition. On the form, you would put yourself, the adoptive parent, under the title of parent. The only other form that asks for the name of the parents would be Schedule A [IMM 5669]. This is to be completed by the principle applicant, aka the person you are sponsoring. This is a form used for all applicants, not only adoptive children, and so the adoptive parents should be named as the parents.
