Family's PR Expiring - Not Met RO

Can Anyone guide way forward as below details on my Earlier Entries in GTA Toronto. While I am planning to Come Ontario GTA for living from Mid Dec-2017.
1. My Family consist Myself Electrical Engineer age 55 Yrs, Wife 50 yrs , Daughter 24 yrs & Son 23 Yrs Computer Science IT graduation now just completed, And youngest Daughter 8 yrs student. Our 1st Entry 31st Jan-2013 upto 1st March-2013, Then again 2nd Entry June-2015 for another 30 Days stay only. Again Left for Home country As my Mother Needs living Help , Lateron She died in same year 2015.
2. My Wife developed some serious Health issues and went through Lobectomy of Lungs from Agha Khan Hospital Karachi, Remain Under treatment for more then a 1 Year , Now improved and Our Family now plans to Rejoin at Toronto , While We lost and Breach the Residency obligation.
Can Anyone Guide way forward to join in Dec-2018 and Also Try Renew PRs of whole Family.
