Hi,
I have the same scenario and i was asked to submit
If you are a permanent resident and changed your name outside of Canada, submit:
- a foreign passport or other national authoritative document that shows your new name; and - > Provided with updated/new name
- an official document linking the old and new names.”
Could you please let me know what kind of official document they are looking for?? An affidavit or something else?
Can you give me an example of this official document?
I am not applicable to update my Provincial ID (photo card or health card) and i need to submit this document within 30 days.
Please help me out. |
