26 years ago my first name was changed from Cristian to CHristian on the canadian citizenship card and consequently all Canadian documents show the same.

The consulate of my country of birth wants a document to show the change in the first name to be able to legalize and change my first name on my birth certificate and other docs issued in the country of birth.

Where could i go and get this document and what is the name of this document?

What would be the procedure and consequences if I want to change my first name back to Cristian instead of Christian?

Hello,

Thank you for sharing your situation and question with us.

You may need to apply for a Legal name change as a way to prove that you have changed your name.

You can find information and the application on the ServiceOntario website.

You may also want to contact ServiceOntario directly at 1-800-461-2156 or 416-325-8305 for more information on this process and to confirm if this is what you will require.

I hope this information is helpful. Please let us know if you have further questions and if there is any follow up to your question/situation.
