We Never Got First Stage - Questions

Hello,
We went the inland spousal route and have thus far received:
1. The letter saying I was okay to sponsor
2. My spouse got their work permit (we asked for at the same time)
3. Many months later my spouse got the medical request saying "your application has gotten the point where we require this information"
However we never actually got a first stage letter? Which we understand is strange as the medical request is near the end of the process?
It's also been many months since that happened and they still tell us on the phone it's PENDING.
Any thoughts as to why they skipped over the first stage approval?

1. The letter stating you were approved to sponsor, was your first stage approval.
