Hi I am writing this for one of my friends, who lost his IMM1000 and got a replacement document IMM 5716.
He does not have a passport or any other photo ID document either from his coutry of origin or from Canada, except Health Card and SIN. He has been a PR since 2001 and he is well eligible to apply for the Canadian Citizenship.
Now he wants to apply for Citizenship and how he can proceed his citizenship application without IMM 1000 and any other IDs like PR card or Drivers License. He cannot apply for PR card due to unavailability of proof of identification or even cannot apply for driving license for the same reason. He has CRA assessment for the year 2012.Please advice me in this regard Thanks

According to the Application for Canadian citizenship under subsection 5(1) – Adults 18 years of age and older (CIT 0002) Guide in the Step 1.Gather Documents section,
You must provide all of these documents in your possession. If any of these documents are no longer in your possession, please explain why.
We suggest that you and your friend contact the Citizenship and Immigration Canada (CIC) Call Centre directly for some suggestions and information regarding this situation.

Thanks Anna for your information. My friend has given his original(IMM 1000) to Onatrio Works, Service Canada for obtaining his SIN Card, Health Card, and getting OW benefits in the past, before loosing his Original. In the past,He had approached OW to know whether they could provide a copy of his IMM 100o from their record but turned down his request, I wonder if he approaches service Canada for a copy of his IMM 1000, would they be able to provide a copy from their records, because they keep a copy of the IMM 1000 to issue SIN card, Please advice

1. CIC no longer issues replacement copies of documents, only a verification of status which your friend has.

I lost my IMM 1000 and I applied for a replacement of this document. In exchange I received a STATUS VERIFICATION letter (IMM 5716). In this letter it shows I am a permanent resident, the date of entry in Canada, the day I became a permanent resident (same day) but my concern is if I can apply for citizenship with this one. I have all other documents (including PR card, Driver's licence, Health card etc). This Status Verification form witch I received can replace the IMM 1000 form?
