OHIP Reimbursement

Hello,
My work permit expires in a week but my renewal application has been approved. However, it will take 6 weeks for the new work permit to reach me.
I have been informed that if I get the new work permit within 90 days of the old one expiring, OHIP will reimburse me for any insurable costs that I incurred while I was waiting for the permit to be processed.
Could someone please provide details (perhaps a weblink) on how and where to apply for the reimbursement?
I am currently pregnant and have to go for lab tests and OB visits quite frequently. I would like to know what information I need to provide or get from the hosiptal/doctors in advance.
Kindly advise.
Thanks.
Ayesha
