Hi
Few questions regarding submitting documents for my parents

1) along with my d.o.b , do I have to submit my parents D.O.B too?
2) if so , what about if my dad's D.O.B doesn't match from birth certificate with all other documents e.g. Passport, driver licence, school living

3) will affidavit works if he notarized that he has been using passport d.o.b on his entire life

Hello,

Thank you for sharing your question and situation with us.

According to the Citizenship and Immigration Canada (CIC) Call Centre, you need to include whatever documents are requested in the document checklist. If it requests your parents' birth certificates, then you need to send them.

You may want to contact the the Citizenship and Immigration Canada (CIC) Call Centre directly for information regarding your application.

I hope this information is helpful. Please let us know if you have further questions and if there is any follow up to your question/situation.
