payment receipt for PR card renewal

is the payment receipt for PR card renewal changed this year comparing to last year? I made the payment online yesterday and printed the receipt. The receipt doesn't have room to fill out address, birthday, phone #, etc. But the receipt my friend made last year had to fill out those info.
