Applying for OHIP - Questions

Hi,
I am currently a full time employee holding a 3-year work permit that is still valid for more than 2 years. I check on your website and found that I am eligible for applying the OHIP.
My company only purchased the additional insurance on top of OHIP, so I need it for my visit to the hospital and etc.
Can you please let me know where can I apply for the plan and what document do I need to apply?
