My purse was stolen while on vacation in the US and my Health Card and Ontario Photo ID card has been stolen. Ive tried calling several numbers provided and it does not permit me to call any toll free numbers because I am in the US.

Hello,
Thank you for sharing your situation and question with us.
We can appreciate that you would be concerned about this situation.
Regarding this type of situation, ServiceOntario recommends that you should also contact your local police department to prevent any fraudulent use of your personal identification. Be sure to obtain the report number and the badge number of the officer who takes the report.
Regarding your Health Card, you mentioned that you are unable to get through the toll-free number for Service Ontario.
In this case, you may want to try and see if it is possible to use the Toronto number here:
416-314-5518 Core Hours: 8:30am - 5:00pm
It is likely that long distance charges will apply if you are able to get through.
In terms of your Ontario Photo Card, to replace a lost, stolen or damaged Ontario Photo Card, you must complete the Ontario Photo Card application form available at a ServiceOntario centre and provide proof of:
- legal name
- date of birth
- signature
The cost for this service is $10.
I hope this information is helpful. Please let us know if you have further questions and if there is any follow up to your question/situation. ===== Anna Settlement.Org Content and Information/Referral Specialist, CIRS Settlement.Org
