Mistake in my DOB from the CIC officer

Hello,
The officer made a mistake in my DOB on the official document how can I correct it.
Also, I've moved to a new address , kindly advice how can I change my address and please provide a phone number if this would be doable on the phone.

Here is an excerpt,
If there is an error on your immigration document:
An amendment will only be made if your document does not match the personal information in your entry passport.
