IMM1000 - Name Spelled Wrong - 50 years Ago

My IMM 1000 has my name spelled wrong, i am trying to get my health card renewed but they wont accept my IMM 1000 because it does not match my current I.D. every piece of I.D. i have has my correct name, Photo I.D. sin card,original red and white health, and everything else.

What form do i need to correct spelling mistake?

You can find some information regarding this type of situation on the Immigration, Refugees and Citizenship Canada website in the Request to Amend Record of Landing, Confirmation of Permanent Residence or Valid Temporary Resident Documents (IMM 5218) section.

It states that,

Your Record of Landing (IMM 1000) or your Confirmation of Permanent Residence (IMM 5292 or IMM 5688) will be amended only to correct errors made by Canadian immigration officials in recording the information you provided when you applied to come to Canada.



You may want to try and contact the Immigration, Refugees and Citizenship Canada (IRCC) Call Centre directly to see if they possibly have any information and suggestions regarding this.
