PR card renewal for minor child- Signature

HI
I am applying for renewal of PR card for my daughter first time and need help with few questions-
1) There are two boxes in item G (i.e Question 25) of application form IMM 5444. One box is for "signature of declarant" and another box says "signature of parent or legal guardian, if applicable".
Do I need to sign in both boxes OR only in one box which says " signature of parent or legal guardian, if applicable"?
2) Item 20 of renewal application form IMM 5444 require work & education history of last 5 years? Do I require to break up the education for each grade year and show it separately with vacation period, OR I can write it in one line showing entry to Junior KG and current date( for grade 2). All the grade classes are in the same (one) school (i.e. no change in school name/ location).

Thank you for sharing your question and situation with us.
According to a representative at IRCC's call centre, you as the parent/guardian should sign for your child as both declarant and parent/guardian. In item 20, they also suggest that you break-up each year "from" and "to" separately even if the school attended is at the same location.
You may still wish to contact the Immigration, Refugees and Citizenship Canada (IRCC) directly for some information specific to your situation. You can contact them by calling 1-888-242-2100 (In Canada Only) or by web form. Hi Harpit,
Did you sign both boxes? My kid is 8 years. Do we need to sign both or one box(first or second box)
