It states,
You may NOT sponsor if you…
[...]-receive social assistance for a reason other than disability,
So as you can see it states that you may not sponsor if you receive social assistance for a reason other than disability.

According to the Service Canada website in the Social Insurance Number – Overview section,
Service Canada is now issuing SINs in paper format (confirmation of SIN letter). Production of the plastic SIN card has stopped, however, SIN cards that are not expired and are currently in circulation can still be used.
You may want to contact the Social Insurance Number Program directly for some information.

Regarding your question,
Is it to late to go through family doctor?
You may want to try and contact a family doctor, however, unfortunately, because you will not have completed the three-month waiting period for OHIP, you will likely have to pay for services.
Regarding your second question,
Are there any private hospitals to give birth?
You mentioned that you are returning to Toronto.
Many of the hospitals have "Family Birthing Centres" or "Birthing Units". There is also a Toronto Birth Centre which is midwife-led and was planned and designed in collaboration with midwives.
For use of the hospital "Family Birthing Centres" or "Birthing Units", as mentioned above, unfortunately, because you will not have completed the three-month waiting period for OHIP, you will likely have to pay for services. Canada’s entry requirements are changing
Permanent residents (PR) of Canada must carry and present their valid PR card or permanent resident travel document (PRTD) when boarding a flight to Canada, or travelling to Canada on any other commercial carrier. If you do not carry your PR card or PRTD, you may not be able to board your flight, train, bus or boat to Canada.
It is your responsibility to ensure that your PR card is still valid when you return from travel outside Canada, and to apply for a new PR card when your current card expires.
Returning by private vehicle
There are other documents you can use to enter the country.
When you return to Canada, apply for a PR card if you plan to travel outside Canada again.
Some examples of private vehicles include, but are not limited to: a car, truck, motorcycle, or recreational vehicle that you own, borrow, or rent, and that is not available for public use.
Returning by commercial vehicle: airplane, bus, train, or boat
You must apply for a permanent resident travel document (PRTD), valid for one entry. Otherwise you may not be able to travel to Canada.
You can apply to replace your PR card when you return to Canada.
