You can find some information on the Ministry of Health and Long-Term Care website in the OHIP Coverage Waiting Period section.
Here is an excerpt about who is exempt,
Is anyone exempt from the waiting period?
Yes; these include, but are not limited to:
- newborn babies born in Ontario;
- OHIP-eligible adopted children under the age of 16;
- protected persons (Convention Refugees and persons in need of protection); and
- people from another province/territory who move directly into a long-term care facility in Ontario, or who require admittance into a long-term care facility within 3 months of arrival in Ontario.
As you can see, returning Canadian Citizens would most likely be subject to the waiting period.

Thank you for sharing your situation and question with us.
We have contacted some Immigration, Refugees and Citizenship Canada Call Centre Representatives.
They mentioned that you do not need to do anything regarding the work permit. Once you become a Permanent Resident, the Work Permit will become invalid.
If you get a divorce during your sponsorship application process, this will change the conditions of your sponsorship application. You will no longer meet the requirements for spousal sponsorship.
If this happens, it is best that you contact the Immigration, Refugees and Citizenship Canada Call Centre for some information regarding your status.
Something else you need to keep in mind is your Social Insurance Number (SIN).
SINs that begin with a "9" are issued to temporary workers who are neither Canadian citizens nor permanent residents. It is likely that yours starts with a "9". Once you become a permanent resident, you will no longer be a temporary worker.
You can find some additional information on the Service Canada website in the Social Insurance Number that begins with a “9” section.
Here is an excerpt,
SINs that begin with a "9" are issued to temporary workers who are neither Canadian citizens nor permanent residents.The use of these SINs is temporary and are valid only until the expiry date indicated on the immigration document authorizing them to work in Canada.
If your SIN begins with a "9", you must update your SIN record to ensure that the expiry date always corresponds with the expiry date on your document from Immigration, Refugees and Citizenship Canada authorizing you to work in Canada. Once your SIN record has been updated, you will receive a SIN with the new expiry date. Your previous SIN (whether it is a paper format or plastic card) is no longer valid and should be destroyed in a secure manner.

According to an IRCC Call Centre representative and the Applying for a Permanent Resident Card (PR Card) - First application, replacement, renewal or to change sex designation (IMM 5445) Instruction Guide, you are required to provide " One (1) main identity document
A photocopy of one of the following:
-your valid passport or travel document or -the passport or travel document you had when you became a permanent resident (including the passport page that was stamped when you arrived in Canada and became a permanent resident) or -the certificate of identity or travel document issued by the Minister of Immigration, Refugees and Citizenship Canada or a foreign country.
It also states that,
The copy should show:
-the document type and number; -issue and expiry date; -your name; -your photo; and -your date of birth
In terms of notarization it states,
You must send the following for any document that is not in English or French:
-the English or French translation; and -an affidavit from the person who completed the translation (see below for details); and -a certified copy of the original document.

"
The affidavit must be sworn in the presence of:
-In Canada: -a notary public -a commissioner of oaths -a commissioner of taking affidavits
