Hey Rashpal,
I am in a similar situation. In my case, my uncle (father's younger brother) adopted me when I was around 12. Now my biological sister has moved to Canada and I want to gain 15 points by adding her in my profile. I can get the birth certificates as a proof but my worry is that my actual application and passport have my uncle's and aunt's name as parents.
Could you guide me on all the documents that I need to get to submit as proofs.

I would suggest you too confirm the same. Once you get ITA explain your situation transparently with the twitter reply in LOE. I am providing reply on my tweet.
