Thank you for sharing your situation and question with us.
You and your spouse can find some information on the CIC Website in the Guide 3900 - Sponsorship of a spouse, common-law partner, conjugal partner or dependent child living outside Canada guide.
It states,
You may NOT sponsor if you…
[...] -receive social assistance for a reason other than disability,
So as you can see it states that you may not sponsor if you receive social assistance for a reason other than disability.
It may be helpful if your spouse contacts the Citizenship and Immigration Canada (CIC) Call Centre directly for information specific to your situation.
In terms of getting the Canada Child Tax Benefit (CCTB), you must meet all of the following requirements listed on the Canada Revenue Agency (CRA) website.

Are you eligible?
To get the CCTB, you must meet all of the following conditions:
-You must live with the child, and the child must be under 18 years of age. -You must be the person primarily responsible for the care and upbringing of the child.
Note If a child does not live with you all the time, see Do you share custody of a child? You must be a resident of Canada for tax purposes. We consider you to be a resident of Canada when you establish sufficient residential ties in Canada. For more information, see Income Tax Folio S5-F1-C1, Determining an Individual's Residence Status.
You or your spouse or common-law partner must be: -a Canadian citizen; -a permanent resident (as defined in the Immigration and Refugee Protection Act); -a protected person (as defined in the Immigration and Refugee Protection Act); or -a temporary resident (as defined in the Immigration and Refugee Protection Act) who has lived in Canada throughout the previous 18 months, and who has a valid permit in the 19th month other than one that states "does not confer status" or "does not confer temporary resident status." If this is your situation, do not apply before the 19th month.
It additionally states about the statement Primarily responsible for the care and upbringing of a child,
Primarily responsible for the care and upbringing of a child – means that you are responsible for such things as supervising the child's daily activities and needs, making sure the child's medical needs are met, and arranging for child care when necessary. If there is a female parent who lives with the child, we usually consider her to be this person.

Regarding the list of Goods to Follow, once you arrive you must provide this list to a Customs Officer at the time of landing.
With respect to the forms, they are both correct and you should use both. The first one:
B4 Personal Effects Accounting Document
This one provides 8 lines for a list of goods that you are bringing with you. If there are not enough lines it is possible to use the second form (below) as well. However, goods that you are bringing and goods to follow should be listed separately.
B4A Personal Effects Accounting Document (list of goods imported)
This form can be used as a Goods to Follow list. There is a check box on the right hand corner where you can check off that it is the Goods to Follow list.
Regarding your question about the Importer's Address, it must be a Canadian address.

Foreign Workers and Their Families
-You have a work permit that is valid for at least 6 months. You also have a formal agreement to work full-time for an employer in Ontario. The agreement states the employer's name, your occupation, and states that you will be working for at least 6 consecutive months.
-You have a valid work permit under the federal Live-In Caregiver program.
-You hold a valid work permit under the federal Seasonal Agricultural Worker Program.
-You are a foreign clergy member who will serve a religious congregation in Ontario for at least 6 months.
-Your spouse or same-sex partner is a foreign worker who is eligible for OHIP.
-You are the dependent child of a foreign worker who is eligible for OHIP.
So as stated, you must have a work permit that is valid for at least 6 months and also have a formal agreement to work full-time for an employer in Ontario. The agreement should state the employer's name, your occupation, and state that you will be working for at least 6 consecutive months.
It may be best for you to contact the Ministry of Health and Long-Term Care Infoline directly.

Thank you for sharing your situation with us.
If you are referring to the conditional permanent resident status, it only applies to partners if their spousal sponsorship applications and spousal sponsorship applications that were submitted on or after October 25, 2012 AND:
*they were in a relationship for 2 years or less when they applied; and
*they did not have any children together when they applied.
Here is some additional information regarding losing your permanent residence from the CIC Website,
Losing your permanent resident status
Losing your permanent resident status does not happen automatically. You cannot lose your permanent resident status simply by living outside of Canada long enough that you don’t meet the residency requirement. Unless you have gone through an official process, you have not lost or given up your permanent resident status, even though you may not be eligible to return to Canada as a permanent resident.
You may lose your permanent resident status if:
An adjudicator determines that you are no longer a permanent resident following an inquiry.
A visa officer determines you do not meet the required residency when you apply for a permanent resident travel document or temporary resident travel document.
You may lose your permanent resident status in one of the ways described above if:
*you do not live in Canada for two out of five years;
*you are convicted of a serious crime and told to leave Canada; or
*you become a Canadian citizen.
You do not lose your permanent resident status if your PR card expires.
In terms of filing marriage fraud against your husband or having your husband deported, it is not likely.
Regarding deportation, if you are a permanent resident and are convicted of a crime you could lose your permanent resident status, be deported and might not be able to come back to Canada.

Here is an excerpt,
Application of Policy Amounts received as compensation for pain and suffering and expenses actually and reasonably incurred or to be incurred as a result of an injury to or death of a member of the benefit unit, including any prejudgment interest, are exempt as assets up to a maximum amount of $25,000 per award.
Amounts received under settlement agreements and other awards, other than amounts paid for loss of income, may be partially or fully exempt as assets as outlined in regulation.
An award received by an applicant or recipient must be verified by the caseworker in order to make a determination of eligibility for an exemption.
Interest (other than prejudgment interest), dividends, or investment income from awards for compensation are considered income. Any income gained on a conversion of assets in excess of the exempted amount is charged as income unless the funds are used to purchase an exempt asset.
You may also want to speak directly to an Ontario Works Case Worker directly for some more information.

According to a ServiceOntario Representative, the 12-month period begins when you leave Canada.
Therefore, it is likely in your case that it would have begun March 2015. Therefore, it would seem that you would have until March 2016 to meet the eligibility criteria of "be physically present in Ontario for 153 days in any 12-month period".
However, we strongly suggest that you call ServiceOntario directly to confirm this information and for some information specific to your situation.
