Do I need the Medical Surveillance for PR Card?

Dear Moderator.
We received our landing visa and i got my medical surveillance form as well. We will be landing in August and will be staying only for three weeks, is it possible to do the medical surveillance after 2 years? What should i need to do? Is this medical surveillance a requirement for me to have my PR card?
